Request for Report

When we are asked to give medical information about you in the form of a report, letter or certificate, the request kick starts a series of processes.

This takes time and is not always straightforward or simple to complete. Some of the information is not available easily and will mean practice staff have to sort and select the right information for the request. You may also need to have an appointment with a nurse or doctor.

When we receive a request we need to establish who is funding this work if it is not part of core NHS work.

We receive requests from insurance companies, the DVLA and police

Please note that the minimum charge is £25 and we ask for payment before starting to compile the information.

How long it takes

Please allow a minimum of four weeks for your report to be available. If you require an appointment due to the nature of the request, please allow six weeks.

Not all documents need a signature by a GP, for example passport applications. You can ask another person in a position of trust to sign some documents free of charge.
