You and your general practice (YYGP) has been developed to help patients understand what to expect from their general practice and how they can get the best from their GP team. YYGP also enables patient to provide feedback or raise concerns with their GP Practice, Healthwatch or the integrated care board (ICB).
YYGP describes what practices and patients can expect of each other. Patients are encouraged to be on time, be prepared, and avoid wasted appointments by cancelling early. Patients can ask to be supported by general practices in a variety of ways (for example, by making reasonable adjustments).
Information in the ‘You and Your General Practice’ document is provided and directed centrally and is subject to national review. The practice is taking steps to deliver your GP care in line with it, subject to available staffing, equipment, software and other physical, legal, contractual and safety constraints, as well as central factors and resources outside the control or gift of the practice.
Patient safety and safe working for practice staff means that we are unable to have all online forms available all day during working hours. All request for GP\ANP appointments made via an online form and medication enquires are considered urgent until they have been triaged.
Please note that The Weardale Practice is open from 8am to 6pm.
