The Importance of keeping your information up to date.

The practice usually has one, some or all of the following information on your patient record.

  • Address
  • Home telephone number
  • Mobile telephone number
  • Alternative contact (could be partner, parent etc)
  • Work number
  • Email

Please be aware that it is your responsibility to let the practice know if there is any change to your contact information.

Please also be aware that practice IT systems and hospital systems are not the same so if you update your details in any way, you need to do this for each hospital consultant or other service that you may be under.
