Are you a Carer?

The Weardale Practice are in the process of reviewing patients who are a Carers. Patients will be contacted over the next few months to update our records.

To support this project a questionnaire has been added to the website which enables patients to update the information themselves.

Please click here, navigate to Admin request, click on Other admin request and select Carers.

The website now has a dedicated navigation option on the homepage for Carers to access useful information – this page is currently being developed. Carers will be notified when the page is live.
