Health and Care organisations must make reasonable changes to their services so they can be accessible to patients with disabilities. This includes people with a physical, sensory, learning, or mental disability, including dementia and autism.

The Weardale Practice are starting to send questionnaires to patients (or carer) on our learning & disability register, patients with autism, and patients who have dementia.

For any patient with disabilities or their carers, you can tell us about any reasonable adjustments by downloading this form and return it to reception and either Wolsingham, Stanhope or St Johns Chapel. Click here to download form or email it to stanhope.practice@nhs.net
