Following the announcement 5 February that we were asking patients to complete an online form for routine new and existing medical conditions, we have temporarily suspended the new process.

For patients who completed the forms online, the admin team are contacting those patients to book an appointment. Please note that you may receive a patient invitation to book the appointment if we can not contact you by telephone.
